I understand that the following medications (paracetamol, Ibuprofen) are already provided at camp and will not pack any of them.*

I consent to receive promotional and informational emails from HolyGround until I request otherwise. Supplied personal information will be confidential and pursuant to the privacy act, and will only be used by the NEM team to effectively care for this camper and not used or any other purposes.*

I consent for this camper to be transported to/from camp by an NEM approved volunteer driver if a ride has been requested or off-site activities are planned. If I do not consent I will discuss this with the HOLYground team.*

I agree for this camper to attend and fully participate in this camp, understanding that while some activities may entail an element of risk, steps will be taken to minimise any risks to this camper.*

In the event of illness or injury, I consent for this camper to receive such treatment as is deemed appropriate by a delegated camp first-aid staff. E.g: wound care, paracetamol.*

If further treatment is required and the emergency contact is unable to be contacted, I consent to this camper receiving required treatment until the emergency contact can be contacted.*

I understand that any images of this leader obtained during camp may be used in the promotion of camp through the HolyGround website, printed publications, camp videos and social media. If there is a reason I do not consent to this, I will contact the camp team to discuss my needs.*

In confirming, I agree to help this camp run smoothly. I understand that I may be sent home if I am unable to follow camp rules or am negatively impacting the experience of others.*
